Second Chance Home provides transitional housing and support services to teenaged mothers and their children who are "homeless," as defined by the U.S. Department of Housing and Urban Development, as follows:

(1) An individual who lacks a fixed, regular, and adequate night time residence; and
(2) An individual who has a primary night time residence that is:
  (A) a supervised publicly or privately operated shelter designed to provide temporary living accommodations (including welfare hotels, congregate shelters, and transitional housing for the mentally ill);
  (B) an institution that provides a temporary residence for individuals intended to be institutionalized; or
  (C) a public or private place not designed for, or ordinarily used as, a regular sleeping accommodation for human beings.

Residents must also meet the following eligibility guidelines for our main facility:

arrow Parent must be single and 14 - 17 years of age.
arrow Parent must have 1-2 children and legal custody of their child or children, or must be in the third trimester of pregnancy.
arrow Children must be 6 years old or younger.
arrow Parent must be a ward of Erie County or New York State.
arrow No active use of alcohol or drugs.
arrow If there is a history of alcohol or drug abuse, the applicant must be in treatment or rehabilitation, or have verification that it is not needed.
arrow No history of abuse toward staff.
arrow If there is a history of abuse toward children and Child Protective Services is involved, parent must be willing to comply with recommended mandates.
arrow Be willing to become a caring parent and to provide care for the child; interested in planning for child’s future.
arrow Be interested in personal growth as demonstrated by willingness to participate in counseling, support groups, educational seminars, furthering education, obtaining job skills, or vocational training.
arrow Willing to learn and perform life management skills; including budgeting, nutrition, cleanliness, personal hygiene, housekeeping, time management, health and dental.
arrow Respectful of others by demonstrating characteristics of dependability and trustworthiness.
arrow Both applicant and child (ren) must have had completed medical examinations in the last 60 days or made an appointment for treatment/exam, testing for contagious diseases.
arrow If applicant is suffering from any known illness, they must continue under the care of a physician and take prescribed medication.
arrow If the applicant is suffering from a mental illness, he/she must have regular psychiatric care, be taking any medication that may be prescribed, be stabilized and have the ability to function in a group home setting.
arrow References from case worker, school, other human service or religious professionals.
